uhd: mpm: apply clang-format to all files
...
Applying formatting changes to all .cpp and .hpp files in the following
directories:

Also formatted host/include/, except Cpp03 was used as a the language
standard instead of Cpp11.

Formatting style was designated by the .clang-format file.

uhd: Fixed multiple compiler warnings

- Use ULL and LL etc. so compiler doesn't need to decide constant's types
- define the empty 'while' loop to include {} to make older compilers happy
- Use explicit type names for BOOST_CHECK_EQUAL
